Here are some quick guidelines:

• Start all pages with a second-level heading (==). Never use first-level headings (=).
• Don't skip heading levels. Use the fourth-level heading only after using the third-level heading.
• Text following any headings should be indented once (:), which means that all text should be at least indented once.
• Also indent lists (*) following a heading.
